Changes:
- Updated the `update-or-create-contact-and-account` helper to support a
new input: `psychologicalStageChangedBy`, and to set the provided value
on contact records.
- Updated `save-questionnaire-progress` to set a
psychologicalStageChangedBy value when updating contact records.
